One of the benefits of moving to SystmOne is that we can be part of the Derbyshire Shared Care Record (DSCR) project. This page explains:

What is the Derbyshire Shared Care Record (DSCR)?

Historically, health and social care organisations in Derbyshire held different sets of records about you. The information in different records may be duplicated or incomplete.

The DSCR is a confidential computer record that joins up different records to create a more comprehensive and up-to-date record about you - this will help improve the care you can deliver to patients.

How do I access the DSCR through SystmOne?

Most clinicians will already have access to the DSCR via SystmOne.

Derbyshire Shared Care Record icon To access the DSCR from SystmOne, you will first need to open a patient record. From there, click on the icon on the top button bar that shows a blue and green globe and says 'DSCR' underneath. 

New starters may need to wait up to a week after they first log into SystmOne to gain access to it however this should be enabled automatically.


How do I access the DSCR outside of SystmOne?

If you are a clinician working in a service that does not use SystmOne, or if you want independent access to the DSCR, there is a form to complete in Microsoft Forms. The form includes some training in the use of the DSCR that you must complete before you can request access.

By completing the training and submitting the form, you will trigger an email to the support organisation to generate an account which would provide direct access to the system.

Accessing patient records: for legitimate reasons only

Please remember you need a legitimate reason to access patient records, such as being part of that person's current care team. In some cases you will be prompted to record a reason why you've needed to access the record. This forms part of the patient record and audit trail. This alerts care teams and tells them why you have accessed the record, and it will be available for subject access requests if the patient wishes to view their own record.

You must not be nosey and must not snoop. Viewing a record without a legitimate reason would be a breach of confidentiality, and against Trust and national policy. This may result in disciplinary action and potentially made public via court proceedings, with financial and reputational impact.

Scenarios that would be considered a breach of confidentiality and Trust policy include:

  • If you or a family member have currently or previously had care provided by the Trust, you must not access your own patient record or your family member's patient records
  • Likewise, you must not access records of friends, neighbours, acquaintances and work colleagues
  • You must not ask other colleagues to view records on your behalf, or share records with you, where you do not have a legitimate reason (such as one of the conflict of interest examples shown in the bullet points above)
  • You must not share computer accounts or use a colleague’s account to inappropriately access patient records. This is against Trust policy and the Computer Misuse Act.

Information governance

A data sharing agreement has been completed for all health and social care organisations to enable sharing of data for direct care.

The NHS has released a statement to support the sharing of data for direct care which can be reviewed in full on YouTube.

“Whilst it is right to be diligent about confidential data, this should never come at the expense of people’s care. We are calling on staff to prioritise the availability of relevant information at the point of care so that individuals can receive the safe and appropriate treatment they expect.”

How do I learn how to use the DSCR system?

The Joined Up Care Derbyshire website includes a DSCR user guides and information page. As well as quick reference guides, there are a series of video guides on YouTube - you can also view these videos at the bottom of this page.

Access to the DSCR is governed by Role Based Access Codes (RBAC) so that professionals can only access information that is appropriate to their job. Training is dependent upon the level of access that you have been given within the RBAC model.

What is the DSCR?

The DSCR brings together information from Derbyshire NHS and Social Care organisations so that staff caring for a patient can see more information in one place. This is viewable by all health and social care staff in Derbyshire via their local systems.

How do I access the DSCR?

Staff can access the DSCR from their local systems when viewing a patient/citizen's record. A full list of systems can be found on the Joined Up Care Derbyshire website. You should have received a welcome email from the DSCR Admin Team recently which provides further instructions.

Do I have to enter information in to the DSCR?

No, currently the system is ‘read only’ to provide you with more information when caring for patients. You will continue to enter patient information in your local system as you do now. 

If a patient asks to ‘opt out’ of having a DSCR record, what do I do?

Please direct the patient to the Joined Up Care Derbyshire website where more information is available on the DSCR and how to opt out of having a DSCR record.
